Clinical and Economic Impact of Community-Onset Urinary Tract Infections Caused by ESBL-Producing Klebsiella pneumoniae Requiring Hospitalization in Spain: An Observational Cohort Study

Objective: To analyze the clinical and economic impact of community-onset urinary tract infections (UTIs) caused by extended-spectrum beta-lactamase (ESBL)-producing Klebsiella pneumoniae requiring hospitalization. Methods: A retrospective cohort study that included all adults with a UTI K. were admitted to tertiary care hospital in Barcelona, Spain, between 2011 2015. Demographic, clinical, data analyzed. Results: One hundred seventy-three episodes UTIs studied; 112 non-ESBL-producing 61 ESBL-producing. Multivariate analysis identified ESBL production, acute confusional state associated UTI, shock, time taken obtain adequate treatment as risk factors for failure during first seven days. An showed differences ESBL-producing total cost hospitalization per episode (mean EUR 6718 vs 3688, respectively). higher costs found statistically significant production treatment. Conclusion: antimicrobial therapy are worse outcomes.
